Documents you need

• ✓## 1. Passport & Personal Documents
• ✓**Original Passport:** Minimum 7 months validity with at least 2 blank pages; previous/old passports required if available; copies of visas from countries visited within the last 3 years and latest China visa, if applicable.
• ✓**Photograph:** 1 recent passport-size photo with white background, without glasses and without wearing white clothing.
• ✓**Application Form:** Information form will be provided by us and must be completed with the required details.
• ✓**National ID (NID):** Clear copy of the applicant's National ID Card.
• ✓## 2. Medical Documents
• ✓**Invitation Letter from China:** Official invitation letter from the Chinese hospital, arranged after payment of the required hospital deposit.
• ✓**Medical Reports from Bangladesh:** Current doctor's prescription and relevant diagnostic/medical reports in clear scanned copies.
• ✓**Suitability / Fitness Letter:** Suitability letter or Fit-to-Fly Certificate may be required depending on the applicant's circumstances.
• ✓## 3. Financial & Professional Documents
• ✓**Bank Statement & Solvency Certificate:** Last 6 months' bank statement and bank solvency certificate with official bank stamp and minimum balance equivalent to USD 10,000.
• ✓**Proof of Profession:** NOC, Trade License, Student ID Card, or Visiting Card, as applicable.
• ✓**Proof of Relationship:** Marriage Certificate required for a spouse attendant; not required if spouses' names appear on each other's passports, while other attendants require a Family Relationship Certificate.
• ✓**Name Mismatch:** A Magistrate-notarized affidavit on BDT 300 stamp paper is required if there is any name mismatch across documents.

Important

Hospital Deposit Hospital Deposit: Equivalent to RMB 3,000 in Bangladeshi Taka. The hospital deposit will be adjusted against the patient's hospital bill after arrival in China, subject to the hospital's terms and conditions.
